Immunization Compliance Policy

Submit Your Immunization History Securely Online

Oklahoma state law requires all new students attending Oklahoma colleges and universities for the first time provide proof of immunization for a full series of:

  • Hepatitis B
  • Measles-Mumps-Rubella (MMR)

A meningitis vaccine is strongly encouraged (but not required) if you are living in campus housing.

If this requirement is not met, an enrollment hold will be places on the student's account during the second week of classes. A student may not enroll in classes for their second semester until the Immunization Compliance has been submitted and verified.

Once records are verified and the enrollment hold has been released from the student's account, it may take up to 2 business days for the hold to clear in OSU's Banner system.

Proof of Immunizations

Proof of immunizations requires the following:

    • Shot records
    • Medical records
    • School health records
    • Laboratory test results demonstrating immunity
  • A printout from your state immunization registry

If you are an Oklahoma resident, you may look up your immunization record at

Failure to comply with these requirements will result in a hold being placed on future enrollments.

  • Medical, religious and moral exemptions are allowed by law and such requests must be made in writing using the OSU Certificate of Exemption form.
  • The requirement shall not apply to students enrolling in courses delivered via the internet or distance learning formats in which the student is not required to attend class on campus.

A PDF version of the Certificate of Exemption is available here. Please fax your signed copy of the completed form to (405) 744-6556.
